- A valid photo ID
- Your private health insurance card
- Any relevant medical records

- Auto accidents: PIP and any available claim information
- Workers’ compensation injuries: Your employer’s insurance details (we specialize in occupational medicine for workplace injuries)
If you’re scheduled for physical therapy, your first visit may take up to an hour. Minors must be accompanied by a parent or guardian for their initial physical therapy visit.
