Excelsia Portal. If you are an Excelsia patient, we may disclose certain health information to your authorized legal representative through the Excelsia Portal. This information may include medical records, billing statements, appointment details, and status updates.

Google Analytics. We may also share non-identifiable information with Google Analytics to track and analyze the use of the Services. We do not use your personal information for profiling in furtherance of decisions that produce legal or similarly significant effects.

Outside of our regular healthcare practices and related healthcare services, all of which are subject to the Notice of Privacy Practices and our obligations under HIPAA, we do not process sensitive personal data as defined under state privacy laws (including data revealing racial/ethnic origin, religious beliefs, mental or physical health diagnosis, sexual orientation, citizenship status, genetic or biometric data, or personal data from known children) without your explicit opt-in consent.

4. SMS/MMS Mobile Message Marketing

By signing up to receive text messages, you agree to these terms and our Mobile Message Terms [HYPERLINK TO SMS TERMS].  We use your mobile phone number to send you text notifications, text marketing offers, and transactional texts, including requests for reviews from us.

The above excludes text messaging originator opt-in data and consent. While we may share information about your opt-outs with third parties to help record your SMS communication preferences, SMS consents are not shared with any third party.

5. Third-Party Websites and Links

The Services may contain links to unaffiliated third-party websites. This Privacy Policy does not apply to the practices of such other websites, and we are not responsible for the actions and privacy policies of the third parties that operate or interact with those websites.

6. Children’s Privacy

We do not knowingly solicit data from or market to children under 18 years of age. By using the Services, you represent that you are at least 18 or that you are the parent or guardian of such a minor and consent to such minor dependent’s use of the Services. However, please be aware that no electronic transmission over the internet or information storage technology can be guaranteed to be 100% secure, so we cannot promise or guarantee that hackers, cybercriminals, or other unauthorized third parties will not be able to defeat our security and improperly collect, access, steal, or modify your information. Although we will do our best to protect your personal information, transmission of personal information to and from our Services is at your own risk. You acknowledge and accept that we cannot guarantee the security of your information.
